PROCESS OF ADAPTATION FOR SPAIN OF THE EUROPEAN FEDERATION OF PERIODONTOLOGY (EFP) LEVEL S3 CLINICAL PRACTICE GUIDELINE (CPG).

Dr Nart, vice-president of SEPA, together with other clinicians, has participated in the Guidelines for the Treatment of Periodontitis stages 1-3, at European level, organized by the European Federation of Periodontology, and published in the Journal of Clinical Periodontology (number 1 in dentistry).

It has also collaborated in its adaptation in Spain by the Spanish Society of Periodontology and Osseointegration SEPA, thus becoming the first medical-legal document in dentistry existing in our country.

WHAT IS THE USEFULNESS OF THE GUIDE AT PRACTICAL LEVELS?

A clinical guide is a guideline or protocol that is useful above all for the clinician and helps to treat all patients in a standardized manner, since it has been observed that if these recommendations are followed there is a general improvement.

The adaptation for Spain of the clinical practice guide level S3 of the European Federation of Periodontology (EFP), based on the existing clinical evidence, is a very effective reference for the dentist with which he/she can continue training and improving at a clinical level as well as enriching his/her knowledge on the subject. In addition, thanks to this, the dentist is able to follow standardized recommendations when treating patients with periodontitis.

These guidelines allow the dentist to know how to act at all times and what treatment should be carried out according to the stage of the periodontitis and help the doctor to know how the patient's disease will evolve in order to prevent possible complications. It has been observed that if the professionals treat in the homogeneous and standardized way provided in the clinical practice guide, according to the scientific evidence, it means an improvement in terms of treatment results.

DIRECT BENEFITS FROM THE USE OF THE GUIDE

The use of the clinical guideline, in this case to treat stage I-III periodontitis, is not only beneficial for the dentist but also for the patient.

On one hand, the clinician is able to obtain good results, since it has been scientifically proven that by following these guidelines the results obtained will be beneficial.

On the other hand, the patient can trust his dentist even more, since he knows what is being done to him at all times and knows how his dentist will act in the next visits, as well as observing the good results of the treatment.

SUMMARY

This adoption of the clinical practice guideline mainly discusses the different steps in the treatment of stage I-III periodontitis and the maintenance phase.

First Section

First of all, it gives a brief explanation about the original work, summary, introduction and clinical relevance, the aim of the guideline, methodology and periodontal diagnosis and classification.

Second Section

Then it talks about the different steps in the treatment of periodontitis and the maintenance phase. In this section there are different clinical recommendations and the methodology used is based on questions.

The last Section

In the last part, it explains the process of adaptation for Spain of the clinical practice guide level S3 of the European Federation of Periodontology.

What specialities are there in dentistry?

Knowing each of the specialties in dentistry or stomatology is very important when we need some attention in our oral health. In this post we will explain superficially what each of the disciplines in dentistry are and what pathologies they treat.

Interdisciplinary dentistry

Restorative dentistry

The restorative or conservative speciality prevents, stops and cures dental caries in all its stages.

SPECIALITY THAT TREATS DENTAL CARIES

Tooth decay can mildly affect the tooth structure where only the enamel is affected or it can progress severely until it reaches the dental pulp and, in the worst case, causing the total loss of the tooth. Periodontics

Periodontics is the speciality that deals with the health of the gum and the alveolar tooth. All patients who have problems with tartar, calculus or anything to do with the periodontium of the tooth, hence its name. Endodontics

Endodontics treats the canal of the tooth, i.e. the inner part of the tooth. It is the speciality that cleans the inside of the tooth or, in the worst case, removes the nerve from the tooth. The cases in which the endodontist usually acts can be diverse, normally treating deep cavities, extreme sensitivity in the tooth or fractures of the tooth that reach the nerve, among others.

Orthodontics and orthopaedics

Orthodontics is the branch of dentistry that deals with correcting the position of teeth.

WHAT TYPE OF PATHOLOGY DOES IT CORRECT?

Although we may have the idea that orthodontics is used for purely aesthetic purposes, the truth is that this type of speciality is used to solve dental alignment problems. Orthodontics is mainly used to align the teeth, which can restore the patient's occlusion, correct crowding, dental rotations, space between teeth, bad bite and a long etcetera. Why it is important to go to the dentist with covid-19

Patients with gum inflammation (periodontitis) are more likely to suffer from other diseases such as poorer control of diabetes, heart attacks and even premature births. People who have suffered from severe periodontitis and have been infected by Covid have had a 9 times higher risk of dying and 3-5 times higher chance of being admitted to the ICU. What are the dentists' risks when dealing with Covid patients?

Dentists have a high risk of Covid-19 infection because they work about 35 centimetres away from patients' mouths. Despite being so exposed, the number of infected clinics and dentists is minimal. By taking the necessary measures to combat the virus, it is much easier to avoid infection. In addition, the dentists, hygienists and assistants of  Nart Dental Clinic strictly use all the necessary protections such as FPP2 masks, gloves and face protection and, for the patient, a rinse with cetylpyridine chloride (CPC) at 0.05% or with chlorhexidine (CHX) before any intervention. 1- We reinforce hand washing.

We reinforce hand washing before and after the patient, first with soapy water and the application of an alcoholic product. The patient is invited to use hand washing with  soapy water and hydroalcoholic gel before and after treatment.

2- Barrier protection methods:

At Nart Dental Clinic we provide our staff with all the necessary EPIS: gloves, protective glasses and screens, hat and work clothes (disposable surgical gowns). In addition, FFP2 masks are used.

3- Rinses with two previous mouthwashes:

Nart Clinica Dental offers a 3% hydrogen peroxide mouthwash and a 0.05% cetylpyridinium (CPC) mouthwash for 1 minute, to decrease viral load.

4- Use of the rubber dam and rotating instruments with anti-return systems.

Its use has been proved by scientific studies that reduces the viral load present in aerosols generated by dental procedures by 70%. The rotating instruments of Nart Clínica have a non-return valve system to avoid the risk of cross infection.

5- Cleaning and disinfection of work surfaces and air.

Disinfection protocols for all work surfaces are reinforced with 1000 mg / L chlorine-based hydrogen products, waiting 30 minutes and then rinsing with clean water. All waste generated from patients is disposed of as medical waste, and is delivered to an authorized supplier. Nart Dental Clinic also has air sterilizers with ultraviolet and ozone technology that works continuously for air disinfection in environments with medical activity.

6- Attention to emergencies  without a companion.

Control of patients in the ward, after telephone triage, maintaining the safety distance is essential to avoid cross infections. PROCESS OF ADAPTATION FOR SPAIN OF THE EUROPEAN FEDERATION OF PERIODONTOLOGY (EFP) LEVEL S3 CLINICAL PRACTICE GUIDELINE (CPG).

Dr Nart, vice-president of SEPA, together with other clinicians, has participated in the Guidelines for the Treatment of Periodontitis stages 1-3, at European level, organized by the European Federation of Periodontology, and published in the Journal of Clinical Periodontology (number 1 in dentistry).

It has also collaborated in its adaptation in Spain by the Spanish Society of Periodontology and Osseointegration SEPA, thus becoming the first medical-legal document in dentistry existing in our country.

WHAT IS THE USEFULNESS OF THE GUIDE AT PRACTICAL LEVELS?

A clinical guide is a guideline or protocol that is useful above all for the clinician and helps to treat all patients in a standardized manner, since it has been observed that if these recommendations are followed there is a general improvement.

The adaptation for Spain of the clinical practice guide level S3 of the European Federation of Periodontology (EFP), based on the existing clinical evidence, is a very effective reference for the dentist with which he/she can continue training and improving at a clinical level as well as enriching his/her knowledge on the subject. In addition, thanks to this, the dentist is able to follow standardized recommendations when treating patients with periodontitis.

These guidelines allow the dentist to know how to act at all times and what treatment should be carried out according to the stage of the periodontitis and help the doctor to know how the patient's disease will evolve in order to prevent possible complications. It has been observed that if the professionals treat in the homogeneous and standardized way provided in the clinical practice guide, according to the scientific evidence, it means an improvement in terms of treatment results.

DIRECT BENEFITS FROM THE USE OF THE GUIDE

The use of the clinical guideline, in this case to treat stage I-III periodontitis, is not only beneficial for the dentist but also for the patient.

On one hand, the clinician is able to obtain good results, since it has been scientifically proven that by following these guidelines the results obtained will be beneficial.

On the other hand, the patient can trust his dentist even more, since he knows what is being done to him at all times and knows how his dentist will act in the next visits, as well as observing the good results of the treatment.
